Client Agreement & Identity Verification

Identity Verification — Required for All Private Clients

All clients are required to verify their identity using a valid government-issued photo ID before participating in a Master Life Blueprint private session. The identity presented must correspond with the person who booked the session and the verified contact information associated with the booking.

Required intake includes full legal name, date of birth, verified email address, verified mobile number, current residential address, emergency contact name and telephone number, and electronic acknowledgment of these terms. Master Life Blueprint should retain only the information reasonably necessary to document verification and administer the service; a copy of an identification document should not be routinely retained unless a legitimate operational or legal need requires it.

Remote-Session Safety

At the beginning of a remote appointment, the client’s current physical location will be confirmed. This permits appropriate local emergency assistance to be contacted if an immediate safety emergency arises. Identity verification is not a background check, medical evaluation, or mental-health assessment.

Scope of Service

Master Life Blueprint private sessions are educational, wellness and personal-development services. They are not psychotherapy, psychiatric care, medical diagnosis or treatment, or emergency/crisis services and are not a substitute for care from an appropriately licensed medical or mental-health professional.

If an immediate safety or mental-health crisis arises outside the scope of the session, the session may be paused or ended and the client may be directed to appropriate emergency, crisis, medical, or mental-health resources. Where immediate danger requires emergency intervention, appropriate emergency services may be contacted.

20-Minute Introductory Focus Session — One-Time Introductory Agreement

One topic. One direct answer. The 20-Minute Introductory Focus Session is a one-time introductory offering available to each individual once. Eligibility is associated with the individual—not with a particular device, browser, IP address, email address, or telephone number.

By purchasing this session, the client confirms that they have not previously purchased, booked, or completed a Master Life Blueprint Introductory Focus Session under another name, email address, telephone number, account, or device. Booking information, verified contact information, transaction records, and related technical records may be used to determine prior use. If records indicate prior use, the introductory booking may be declined or canceled and the individual may instead book an eligible standard private session.

Electronic Consent

Before booking is completed, the client must affirmatively accept the applicable agreement and electronically sign it. The record should include the agreement version, date and time, verified booking identity, and booking or transaction identifier.

Required acknowledgment: “I have read, understood, and agree to the Master Life Blueprint Client Agreement, Identity Verification and Scope of Service terms.”

The Terms of Use and Intellectual Property & Proprietary Rights Policy are incorporated by reference. Additional acknowledgment for the 20-Minute Introductory Focus Session: “I confirm that I have not previously used the one-time introductory Private Focus offering.”
